One of the most common questions we hear in the clinic is: “When can I get back to playing?” Whether you are returning to AFL, Soccer, Rugby, Netball or another sport, it is a reasonable question. The challenge is that returning to sport is rarely as simple as being pain free or ticking off a rehabilitation checklist.

A player may have regained strength but still lack confidence. They may be training comfortably but not yet be ready for the intensity of competition.

Successful return to sport requires us to look at more than just the injury itself. We need to consider the demands of the sport the individual athlete and the level of risk involved.

What is return-to-play decision-making?

Return to play refers to the structured process of determining whether an athlete is ready to safely re-enter training or competition after injury or illness.

It’s a concept recognised across all levels of sport, from community clubs through to elite state competitions. Clinical guidelines from organisations including Sports medicine Australia and The Australian Institute of Sport emphasise that return-to-play decision should be individualised, evidence-informed and made collaboratively between the athlete and their treating clinician.

Why Timing Matters

No single test, pain score, or strength number tells the whole story. An athlete may feel ready long before their body — or their sport — is prepared for the demands of competition.

Why return to play is more complex than it looks

A useful framework for structuring these decisions is the StARRT Framework – Strategic Assessment of Risk and Risk Tolerance. It breaks the decision making into 3 sections.

Tissue health

The first question is: how much load can the injured area currently handle? Key factors include:

  • Pain levels and symptom behaviour during activity
  • Swelling and signs of ongoing tissue irritation
  • Strength deficits compared to the uninjured side
  • Range of motion
  • Imaging results where relevant

A rugby player who has regained full shoulder range of motion after an AC joint sprain may still experience pain in contact positions — a sign that tissue capacity hasn’t fully returned, even if things feel better day to day.

Sport-specific demands

Being pain-free during rehabilitation exercises is not the same as being ready for competition. The demands of your sport must match what your body can currently handle:

  • Contact versus non-contact sport
  • Position played and specific movement demands
  • Training versus match intensity
  • Fatigue and conditioning levels
  • Ability to protect the injured area during play
  • Psychological readiness to return

Consider the difference between an AFL midfielder and a ruckman — both may have the same knee injury, but the rotational loading, ground contact demands, and aerial contest requirements they face are entirely different. A netballer returning from a foot injury faces repeated high-impact landing demands that simply don’t exist in a gym-based rehabilitation programme. A soccer goalkeeper and a central midfielder may share the same hamstring strain, but their return timelines should reflect the very different physical demands of each role.

Risk tolerance

This is often the most overlooked part of the process. Every athlete, coach, and clinician approaches risk differently. Key questions include:

  • Is there a finals series next week?
  • Is this a young athlete with a long playing career ahead of them?
  • Is the athlete prepared to accept a higher risk of reinjury in order to compete?
  • What are the consequences of delaying return?

It’s important to understand that risk tolerance doesn’t change the actual risk — it changes what level of risk we’re collectively willing to accept. A netballer returning for a grand final may accept a higher level of risk than she would three rounds into the season.
